Queen of the North Ferry

The official report of the sinking of this ferry - an accident that should never have happened. You can have all the automation you want, but you can't cover for stupidity.

This sinking got some media attention, but not very much - certainly not enough for what happened.

My wife & I sailed on the Queen of the North on this route in 1998, and noting the many changes in direction throughout the route, I assumed there was an audible/visual signal alert at each way point.
Page 41 of the report points out that the important safety features (alarms) had been either deactivated or silenced.
I'm very surprised that the report has taken so long to be made public.
The Marco Polo incident earlier this year, which result in a fatality, has already been reported on by the Bahamas Maritime Authority and there is to be a coroners inquest in the UK.
I'm guessing that the widow of the 87 year old fatality, may herself be well advanced in years, so I would hope that speedier progress would be made for a final report on this incident.
