This can only be either Apollo or Ariadne due to the cones of the UA13 ESM antenna on the masthead.

This photo is one of three ships, either MINERVA, CLEOPATRA or PHOEBE. It shows a Batch 2 Leander after being fitted with Exocet, but not fitted with towed array, with the aerials for UA-13 at the foremasthead. Only these three carried UA-13 after Exocet conversion (APOLLO, ARIADNE and three Netherlands Leander class ships also carried the array seen at the masthead, but these had different Seacat systems and other differences). The photo appears elsewhere captioned as MINERVA in 1987.
